Joining the Insurance


There are two possibilities:

Online Insurance Enrollment

Online enrollment is a two-step process:

  •  Step 1 – Fill out the application form.

  • Step 2 – Enroll in the insurance on mojePZU: www.moje.pzu.pl or in the mobile app.

ATTENTION! Only declarations sent by the 15th day of the month preceding the protection month allow enrollment in the insurance from the 1st day of the nearest month.

Traditional Enrollment

Simply fill out the insurance enrollment declaration, selecting the desired options. The completed declaration along with consent for premium deduction should be delivered to the person handling the insurance.

Providing written consent for premium deduction from salary: "Consent for premium deduction from salary." Enter your details and your employer's details. Send/deliver the signed document to the person indicated in the contact details in your company to ensure it reaches them by the 15th day of the month preceding the protection month. Lack of consent for premium deduction will result in the insurance not being activated (the employer will not be able to deduct the insurance premium from the employee's salary).

Help in a situation when...

You will fill out the form for the first time

Soon, we will send you an email to the address provided in the form. You will learn from it how to enroll in the insurance.

Log in to mojePZU, go to the PZU Offer tab, and select Group Offer. There you will find the group insurance offer and fill out the enrollment declaration. If you do not remember your mojePZU password, use the "remind password" option or call our helpline.

You have an account in mojePZU and you are filling out the form again – within 3 days

After you submitted the first form, we sent a message to the provided email address. In it, you will find information on how to create an account and check the insurance details.

The insurance program consists of:

 

  • PZU na Życie Plus (PZU For Life Plus),

  • Supplementary Packages:
    • Coverage for a Child package,
    • Support in Case of Neoplastic Disease package,
    • Support after an Accident package,
    • Support for Relatives and Close Ones package – only for the employees
 

Advantages in the new scheme

  • Persons taking out the insurance from 1.06.2025 (and from 1.07.2025 and from 1.08.2025) take out without a grace period (employees, spouses, domestic partners and adult children). The grace period only applies to the Support in Case of Neoplastic Disease Package: 90 days in the event of diagnosis of malignant neoplastic disease, diagnosis of malignant neoplastic disease at an early stage, onset of serious disease, specialised treatment.
  • The benefit for a stay in hospital is due for a stay that lasted at least 1 day due to an accident or 2 days due to a disease. The first day of a stay in hospital is considered to be the date of registration and the last day is considered to be the date of discharge from hospital.
  • In the event of a stay in hospital, we extend our cover to the entire world.
  • Extending the catalogue of the serious diseases of the insuredparty to include the following disease entities: Chronic respiratory failure, Meningioma (irrespective of benign brain tumour), Coronary artery angioplasty, Alzheimer's disease, Paralysis of the limbs due to damage to the spinal cord, Coma, Loss of speech.
  • You may take out the insurance if, on the date on which you sign the declaration of taking out the insurance you are on sick leave, in hospital, on rehabilitation benefit or you are a person declared incapable of working or unfit for service, provided that a continuity of insurance cover is maintained.
  • As a close insured party, you can also take out the insurance without submitting the required health declaration on the date of signing the declaration (in the first 3 months from the date of launching the new insurance scheme, i.e. 01.06.2025, 01.07.2025 and 01.08.2025). This means that we will not pay for a disease that has been diagnosed prior to the start of liability of PZU, but any consequences of the disease, e.g. a stay in hospital, will be covered under the insurance.
  • If the continuity of cover has been maintained for you, we are liable for an event that occurred during our cover, but the cause (disease) of which has occurred during the cover under the previous insurance contract.

Employees eligible for basic insurance include:

  • Company employees aged 16–69,
  • Spouses of employees aged 16–69,
  • Life partners of employees aged 18–69,
  • Adult children of employees aged 18–69.

Employees, spouses, life partners, and adult children aged 16–69 can enroll in additional insurance.

Coverage for a Child package

Benefits of the insurance:

  • Broader financial protection for employees in case of unforeseen events.
  • In the event of a child's health impairment or serious illness (e.g., malignant cancer, neuroborreliosis, sepsis), the parent will receive funds that can be used for any purpose, such as additional medical consultations.
  • If a child is hospitalized due to an accident or illness, the parent will receive a benefit for each day of hospitalization. These funds can help cover costs such as travel expenses to the hospital.

Support for Relatives and Close Ones package – only for the employees

 

Benefits of the insurance:

  • Additional financial support for loved ones in case of the insured's death caused by an accident, traffic accident, or workplace accident.
  • High benefits for a relatively small premium. The employee's loved ones can receive up to PLN 450,100 in case of death caused by a traffic accident at work.
  • The death of an employee often results in the loss of part or all of the family's income. The insurance payout can help the employee's family pay off financial obligations or cover ongoing bills.

Cancer Support Package

  • Broader financial protection for employees in case of unforeseen events.
  • In the event of a malignant cancer diagnosis, the insured will receive funds that can be used for any purpose, such as additional medical consultations or non-reimbursed pharmacological therapy. We provide benefits not only for the diagnosis of cancer but also for treatments such as chemotherapy or radiotherapy.
  • We also offer financial support in case of an early-stage malignant cancer diagnosis, known as in situ.
  • If you want coverage for 29 PLN, you need to choose two premiums of 12 and 17 PLN.

Accident Support Package

  • In the event of permanent health impairment or bone fracture, the employee and their loved ones can receive funds that can be used for any purpose, such as additional rehabilitation.
  • We will provide a benefit to the employee and their loved ones in case of a bone fracture, even if the fracture does not result in permanent health impairment.
